Sustainability and Transformation Plan

NHS England has asked every health and care system to work together to produce a multi-year Sustainability and Transformation Plan (STP), showing how local services will evolve and become sustainable over the next five years and ultimately delivering the Five Year Forward View vision.

Each footprint has to be able to answer the following questions:

· How will we close the health and well being gap?

· How will we drive transformation to close the care and quality gap?

· How will we close the finance and efficiency gap?

As part of this work there has also been a Local Digital Roadmap produced for Coventry and Warwickshire which looks at how we can modernise health and care through the use of technology. You can find out more about this here.
