University Hospitals Coventry & Warwickshire NHS Trust is applying for NHS Foundation Trust status. As an NHS Foundation Trust our patients, staff and members of the public can become 'members' of the Trust. This means you can be involved in shaping the future of your healthcare services. We would welcome you to become a member of our Trust.

Currently we have over 6,500 public members, who receive regular newsletters from the Trust and have the opportunity to join in our Member Events.

To become a member you will need to fill out a membership form. This can be done here online via our secure form.

In order for us to process your application, communicate with you effectively, and to ensure that we build a representative membership, we will need you to tell us a bit about yourself so please do fill out all the sections of the form. All of the information you provide will be kept strictly confidential in accordance with the Data Protection Act 1998 and we will only use this for matters relating to your membership.
