Embryo and Sperm Storage
Embryo storage
This section is for patients with frozen stored embryos
If you currently have embryos in storage that you no longer wish to keep for your own treatment, you have the following options:
1) To dispose of the embryos
If you wish to dispose of the embryos, please complete the following 2 consent forms.
a) Embryo disposal tick-list (EMB-CO-000158V1) confirming your wish to dispose of the embryos
b) Destroy embryos consent form (EMB-CO=000151V2). Please note, to dispose of the embryos we will require BOTH partners signatures and a copy of a form of photo ID (driving license, passport) for each.
2) To donate the embryos to another couple
Before deciding to donate your embryos to another couple, you will need to read the following 2 information sheets:
a) The first (donate couple- embryo donation EMB-PI 000032v3) is our information sheet which explains what is involved for both partners in the process of donation.
b) The second (Donate Couple HFEA Sept07.pdf) explains the legal perspective of donation, including the need to be registered as an embryo donor with the HFEA. Any subsequent children born as a result of this treatment can find out the identity of the embryo donors, via the register when they reach the age of 18
If you wish to donate your embryos to another couple, please complete the following consent form:
a) Embryo disposal tick-list (EMB-CO-000158V1) stating your wish to donate the embryos to another couple
3) To donate the embryos to research
Before deciding to donate your embryos to research you will need to read the following information sheet (donate research EMB-PI-000033V4) which explains what the embryos are likely to be used for if they are donated to research.
If you wish to donate your embryos to research, please complete the following 2 consent forms
a) Embryo disposal tick-list (EMB-CO-000158V1) confirming your wish to donate the embryos to research
b) Donate research (EMB-CO-000034V5)
Sperm storage
This section is for patients with frozen stored sperm
If you currently have sperm in storage that you no longer wish to keep, you can give your consent to have it disposed.
To dispose the sperm you will need to complete the following consent form (destroy sperm and-co-000032v3).
Please note, to dispose of the sperm we will require your signature and a copy of a form of photo ID (driving license, passport).
