This section is for patients with frozen stored embryos and/or gametes. If you currently have embryos or gametes in storage that are due to expire, you can either request an extension of storage or choose to stop storage/remove them from storage.

The consent forms will be provided through our secure electronic consent platform. You will simply need to log in, register, and complete the consent forms online.
