We work closely with organisations across health and social care, including local authorities, the voluntary/community sector and other partners, to meet the needs of our patients.
The Coventry and Warwickshire Health and Care Partnership (HCP) is designing services for the future to meet the additional demands expected from an increasing population that is predicted to live longer.
Together we are doing everything in our power to enable people across Coventry and Warwickshire to pursue happy, healthy lives and put our communities at the heart of everything we do.
This includes transforming services, improving health outcomes and being more efficient in the way we use our resources.
The HCP is working on a five year plan which identifies key local priorities for people in our area and outlines how we will deliver them together.
NHS England has produced a short animation explaining how partnerships between the NHS, local government and the third sector are helping to integrate care and meet health and care needs now and in the future.
