Community Services

From 1 July 2024, Adult Community Health Services in Coventry will be provided by University Hospitals of Coventry and Warwickshire NHS Trust. This follows a review of 'Out of Hospital' services led by the Coventry and Warwickshire Integrated Care Board.

This change means that from 1st July 2024, Coventry & Warwickshire NHS Partnership Trust (CWPT) will no longer be providing these services.

UHCW will bring together a range of community services providing care closer to people’s homes and supporting people live as well and independently as possible.


No, the team and staff members that currently care for you will continue to do so. Their employment is moving to a different organisation, from 1 July they will be employed by University Hospitals Coventry and Warwickshire NHS Trust however will continue to provide your care in the same way.

From 1 July, the following community services will be provided by UHCW.

Adult Community Nursing

Specialist Wheelchairs

Integrated Neuro and Stroke

Urgent Community Response

Podiatry

Long Covid

Community Diabetes and Cardiovascular Risk

Adult Wound Healing (Tissue Viability)

Adult Bowel and Bladder (Continence)

Parkinson’s Nursing

Adult Community Speech and Language therapy

Therapy including Urgent and Reablement, Planned Occupational Therapy and Physiotherapy

Palliative Care

Phlebotomy

Community Clinical Health Psychology

Care Navigation

Integrated Single Point of Access (ISPA)

No. Your appointments will continue as usual, in the same place where you’ve previously been cared for.

You should contact the service to let us know if you cannot make your next appointment, in the same way as you usually would. The contact details for the service remain the same as on your appointment letter. If an appointment has been made for you and you know you can’t attend, it’s important that you let us know as soon as possible.

The change relates simply to the organisation managing the services. Your care will still be delivered by the team and staff that you are already familiar with, and in the same location, whether that’s a clinic, care home or your own home.

Following the decision taken in 2023 by CWPT to no longer be the provider of Adult Community Physical Health Services, UHCW has been awarded the contract to deliver these services for people living Coventry. This means that the current staff who provide the service will transfer to the new provider (UHCW) on 1st July 2024. There will be no change for patients who will continue to access care in the same way as they have previously done.
