Community Services

From 1 July 2024, Adult Community Health Services in Coventry will be provided by University Hospitals of Coventry and Warwickshire NHS Trust. This follows a review of 'Out of Hospital' services led by the Coventry and Warwickshire Integrated Care Board.

This change means that from 1st July 2024, Coventry & Warwickshire NHS Partnership Trust (CWPT) will no longer be providing these services.

UHCW will bring together a range of community services providing care closer to people’s homes and supporting people live as well and independently as possible.

Improving care and support closer to home

As the lead provider for adult community health service for Coventry, we are working with NHS and social care organisations to better coordinate care for people close to where they live.
